Ved at bestille en test accepterer du følgende vilkår og betingelser:

Jeg ønsker at blive undersøgt for Covid-19 / Corona af Amplexa Genetics A/S. I denne forbindelse giver jeg samtykke til, at følgende personoplysninger om mig bliver registreret og behandlet af Amplexa Genetics: Navn, adresse, fødselsdato, telefonnr. og e-mail. Ligeledes giver jeg samtykke til, at oplysninger om, hvorvidt jeg har fået COVID-19, bliver behandlet af Amplexa Genetics. Amplexa Genetics A/S sletter alt personlig information samt information om testresultatet, når det ikke længere er påkrævet os at gemme det, jævnfør journaliseringspligten. Amplexa Genetics er forpligtiget til at indberette resultat af test samt tilhørende personlig og teknisk data til Statens Seruminstitut (SSI) samt Styrelsen for Patientsikkerhed (STPS). Resultatet vil herefter behandles af de respektive offentlige instanser og fremgå på sundhed.dk (for personer med dansk CPR nummer) samt indgå i smitteopsporing, i lighed med tests foretaget i offentligt regi.

Ved positivt resultat:

Jeg er blevet informeret om, at såfremt undersøgelsen af mig viser, at jeg er positiv for Covid-19/Corona, må jeg kontaktes af Amplexa Genetics for opfølgning og indhentning af CPR-nummer hvis nødvendigt.

Rettidig levering af testresultater beror på korrekt e-mailadresse på bestillingsformularen. Fejl i kontaktinformationen vil som minimum medføre forsinket besvarelse. Mindre forsinkelser kan forekomme i tilfælde af tekniske fejl som kræver at testen gentages eller force majeure som midlertidigt umuliggør testens fuldførelse, f.eks. strømafbrydelse.

By ordering a corona test you accept the following terms and conditions:

I wish to be examined for Covid-19/corona by Amplexa Genetics A/S. In connection to this, I give my consent to the registration and handling by Amplexa Genetics A/S of the following personal information regarding me: Name, address, birthday, telephone number, and email. I also give my consent to the handling of information regarding whether I have tested positive or negative for Covid-19. Amplexa Genetics A/S will delete all personal information when it is no longer required to be stored, according to the record keeping obligation under Danish law. Amplexa Genetics are required by law to report test results along with related personal and technical data to Statens Seruminstitut (SSI) and to the Danish Agency for Patient Safety (STPS). The result will be handled by the two agencies and be listed on sundhed.dk (for people with danish social security number) and be included in the infection tracking process, on par with tests carried out by the public.

In case of a positive result:

I have been informed that if the examination regarding me shows that I am positive for Covid-19/Corona, Amplexa Genetics are allowed to contact me for follow up and obtaining my social security number (if relevant).

Timely delivery of test results depends on correctly input e-mail address on the order form. Incorrect contact information will as a minimum result in delayed delivery of test results. Furthermore, test results may be delayed in case of technical errors that require the test to be repeated or instances of force majeure that prevent the completion of the test, e.g. power outage.
